Chimp lip is consistent with linear advancement cases where for the mandible to go 'forward' enough or a lot, so does the maxilla, usually cases where mandibular displacements are prioritized either for airway issues or just significant retrusion. I was at least kinda "cute" before surgery. This is what I've been recommended: maxillary impaciton, with 3-5 mm of advancement, clockwise rotation, and genioplast with BSSO to center. The bigger the advancement, the steeper your occlusal plane, the more convex it will look. In a typical surgical plan, the nose advances 3-4mm so only 3-4mm is added to the upper lip which also thins somewhat. She also moved the upper jaw down so as to show more teeth (said my teeth were too short). So, this very much looks (to me) the issue is PERCEPTUAL CONFUSION and quite possibly one of the 'red flag' perceptual issues where the person views what would be seen by other onlookers as an improvement but views as an aesthetic problem. I realize my tongue moves with the lower jaw so I might still have some posterior anterior constriction but at least there would greater space at the back. Yet want a reversal of it. Will 6mm movement in the maxilla cause… Until we figure out how to lengthen the ramus at the same time and how to advance the rest of the face to match, chimp look will always be a concern.
